IMPORTANT: You will not be scheduled until you complete this form AND hear back from us to finalize your time and date. Submit your info and preferred dates in this form and we'll follow up via email to finish scheduling.

PAYMENT INFO: If you have purchased a lesson, please add your order number in the form below so we know you've paid. If you've not purchased your lesson prior to filling this form, we will work with you to get scheduled anyway and send an invoice for your lesson(s). If not paid by 10 pm the night before the lesson, your lesson will be canceled automatically.

NO-SHOW FEE - $20

This will be charged to your Cherry Peak Account and will need to be paid to be able to take another lesson.

This includes ALL LESSON TYPES.

We understand that emergencies happen but we appreciate a heads up as our instructors show up to teach so their students should show up as well.

TIMING

Lesson submittals are normally processed in the LATE evening and you will be notified by email and/or text.

6:00 PM IS THE LATEST YOU MAY SUBMIT LESSON REQUESTS FOR THE NEXT DAY.
SUBMITTING A FORM IS NOT A GUARANTEE OF A LESSON. WE HAVE TO VERIFY ROOM IN THE GROUPS.
IF YOU DON'T RECEIVE A REPLY THEN YOU ARE NOT ON THE SCHEDULE.
BE SURE TO CHECK YOUR SPAM FOLDER.

IMPORTANT: DO NOT submit form on same day as lesson is desired. Emails are not checked until LATE evening. If it is not a same-day request submitting the form online is best.
